Local Program Evaluation Planning Guide
The California Department of Health Services, Tobacco Control Section prepared this evaluation-planning guide to assist its local programs that are using objectives from the core indicators in the American Cancer Society’s Communities of Excellence in Tobacco Control: A Community Planning Guide (CX). The guide provides an introduction to CX, information on needs assessment, how to identify priority populations, and module 4 includes sample evaluation plans that can be adapted for local programs. At least one sample objective, along with a corresponding sample evaluation plan, is provided for each of the core indicators in the CX.
